PLANNING PHASE

The Planning phase should feel deliberate, structured, and intake-oriented, ensuring all the groundwork is set before the veteran formally “embarks” on their journey in the program.

Objective: Establish a foundation by gathering essential background information, preparing resources, and setting the stage for personalized veteran support.

Core Activities:

  1. Veteran Intake Interview
    a Collect personal background

    b Understand goals, expectations, and immediate concerns

    c Establish initial rapport and trust

  2. Document Collection & Verification

    a DD-214 or discharge papers (verify veteran status)

    b Valid identification

    c Social Security card (if available)

    d VA eligibility documentation

    f Joint Service Transcript

    g Employment History

  3. Needs & Risk Assessment

  4. Medical History (chronic conditions, medications)

  5. Mental Health History (PTSD, anxiety, substance use)

  6. Legal Background (criminal history, probation/parole status)

  7. Financial Snapshot (debts, benefits, income sources)

  8. Internal Preparation

    a Assign a case manager to lead coordination and support

    b Open a veteran case file (physical or digital) for tracking progress

    c Design an initial individualized support plan

    d Prepare a living space (cleaning, staging, stocking essentials)

    f Conduct a staff briefing to ensure readiness for arrival
