Embarkation Phase

EMBARKATION PHASE

The Embarkation phase can be thought of as the stage where the veteran is equipped and readied to begin their journey through the program — similar to troops loading and prepping for deployment.

Objective: Prepare the veteran for active participation in the program by addressing immediate needs and establishing a baseline for care and growth.

Core Activities:

  1. Comprehensive Physical Health Assessment: Identify current medical conditions and schedule necessary appointments.

  2. Mental Health Evaluation: Screen for PTSD, depression, anxiety, substance use, and other concerns.

  3. Dental Screening: Identify urgent dental needs and plan treatment as available.

  4. Issuance of Essentials

    1. Provide:

    2. Clean clothing

    3. Footwear

    4. Toiletries and hygiene products

    5. Bedding and linens

  5. Baseline Assessments

    1. Intake questionnaires (personal goals, strengths, support system)

    2. Initial case plan development.

    3. Set expectations and responsibilities.

  6. Orientation

    1. Introduction to facility rules, schedule, available services, and staff.

    2. Tour of the center and assigned room.

  7. Goal Setting

    1. Review and establish SMART program exit goals.

    2. Validate resources and support systems in place to support goals.



Summary Statement:

The Embarkation Phase is designed to bring the veteran into the center, establish residency, conduct required assessments, set an agreed upon treatment plan, and allow the veteran time to adapt to the new environment.
