QUESTION: HOW DO I/MY RELATIVE/MY FRIEND BECOME A RESIDENT OF FIVE STAR

Answer: The steps to apply for residency at Five STAR are as follows:

  • Must be a veteran with an Honorable or General Under Honorable discharge.

  • Must provide a DD-214, picture ID, and a police background check.

  • Complete the Five STAR application

  • Applicant will be interviewed by staff

  • Staff reviews all information and determines best option for the applicant. This could mean a referral to another organization, higher level of care, etc. Accepted applicants will be welcomed and assigned a room.

 

QUESTION: DO YOU OFFER MENTAL WELLNESS/HEALTH SERVICES?

Answer: Our on-site Counselor provides services to Residents. The Counselor can provide limited support to other veterans and active duty personnel (including appropriate referrals).

 

QUESTION: HOW DO I FIND OUT WHAT VA BENEFITS I CAN APPLY FOR?

Answer: The full range of benefits you may be eligible for can be found on the VA website.

 

QUESTION: WHAT IS THE VA MISSION ACT AND HOW DOES IT APPLY TO ME?

Answer: The Act strengthens the VA’s ability to deliver high quality care at VA facilities, virtually through telehealth, and in your community.

 

QUESTION: CAN FIVE STAR PROVIDE FOOD FOR LOCAL VETERANS?

Answer: Yes, individuals must provide picture ID and proof of veteran status. Food bags are provided no more than once a month per individual.
