Passport To Independence

Program Summary

The Passport to Independence is a structured, multi-phase residential program developed by the Five Star Veterans Center to help displaced and at-risk veterans rebuild their lives and achieve self-sufficiency. Grounded in the values of respect, discipline, and purpose, the program provides a supportive, holistic pathway to long-term stability through housing, health services, professional development, and community reintegration.

Modeled after the U.S. Navy's amphibious assault planning concept, the program follows five distinct, progressive phases: Planning, Embarkation, Rehearsal, Movement, and Action—collectively known as the PERMA Framework.

The Five Phases of the Passport to Independence

  1. Planning Phase (Groundwork)
    This phase gathers essential background information on each veteran to develop a tailored care plan. Staff collect key documents such as the DD-214, conduct intake interviews, review physical/mental health history, assess job skills, and evaluate legal or financial challenges. Veterans are assigned a case manager, and preparations are made for housing and documentation.

  2. Embarkation Phase (On-boarding, Orientation and Goal Setting)
    Veterans undergo comprehensive health and wellness assessments, including mental health and dental evaluations. This phase may also include the issuance of clothing and hygiene items. It marks the official entry into the residential support environment and ensures immediate needs are met.

  3. Rehearsal Phase (Habit Formation)
    During this acclimation period, veterans begin participating in activities and appointments. They build routines, meet peers and staff, and adjust to the structured lifestyle at the center. Early mental health treatment, group interactions, and exposure to program expectations help lay the groundwork.

  4. Movement Phase (Preparation for Societal Integration)
    Veterans advance into focused care and planning. They begin receiving advanced therapeutic services, engage in vocational rehabilitation, and develop realistic career or education goals. Long-term strategies for employment, housing, and financial independence are introduced and customized.

  5. Action Phase (Achieve Self-Sustainment)
    In the final phase, veterans secure stable income sources, such as employment or VA compensation. Long-term housing is obtained, a future care plan is activated, and financial systems are put in place. The program concludes with a formal file closure and assignment of a long-term mentorship contact to support sustained success.
