Action Phase

ACTION PHASE

This Action Phase truly represents the execution of the mission, where the veteran leaves the program equipped and empowered to sustain independent living. It’s the culmination of all the support, practice, and growth from earlier phases, now solidified into long-term outcomes.

Objective: Finalize the veteran’s transition into independent living by ensuring income, housing, support systems, and follow-up care are firmly in place. This phase focuses on execution, empowerment, and long-term sustainability.

Core Activities:

  1. Income Stability

    1. Confirmation of earned income through employment or vocational placement

    2. Confirmation and activation of VA benefits and/or disability compensation

    3. Final review of financial plan to ensure sustainability

  2. Housing Secured

    1. Lease signed or placement in permanent housing

    2. Coordination for moving assistance

    3. Ensure furnishings and essential household items are provided

    4. Home readiness check (utilities, safety, proximity to resources)

  3. Care Continuity

    1. Development of a follow-on care plan:

      1. Scheduled medical and mental health appointments

      2. Connection with community support providers

      3. Medication management, if applicable

  4. File Closure & Handoff

    1. Formal case file closure and documentation of program completion

    2. Provide copy of file or summary packet to veteran (optional)

    3. Exit interview to review achievements and identify any last needs

  5. Ongoing Mentorship & Support

    1. Assign or maintain long-term peer mentor contact

    2. Schedule follow-up calls or check-ins at 30-, 60-, and 90-days post-exit

    3. Offer alumni events or optional support group attendance
